New Client Process
Begin Your Counseling Journey with EverRose Counseling & Wellness
If you are an Ohio resident interested in becoming a client, please follow the steps below to begin the intake process. Our goal is to make starting therapy simple, supportive, and organized from the very first interaction.
Step 1: Submit a New Client Request Form
Click the “New Client Request Form” link below and provide your information for review. Once your request has been received, a member of our team will contact you to discuss next steps and determine if our services are a good fit for your needs.
Step 2: Complete Intake Paperwork
After connecting with a member of our team and being approved for services, you will receive an invitation to access our secure client portal. You will be required to complete all intake forms and necessary documentation prior to your first appointment.
Step 3: Schedule Your First Appointment
Once your paperwork has been completed and submitted, you will be able to schedule your Diagnostic Assessment (Initial Counseling Appointment) through the client portal.
Important Intake Requirement
All required documents must be completed and received at least 48 hours prior to your scheduled intake appointment. If paperwork is incomplete or not received within this timeframe, your appointment may need to be rescheduled.

Current EverRose Counseling & Wellness clients can schedule their next appointment by logging into their existing account.
You will be prompted to sign in using the email address associated with your client portal account. Once logged in, you can view available appointment times and schedule your next counseling session.
